Syllabus: | This unit describes the role of the pharmacist in the development and provision of disease state management services in community pharmacy. Students will learn how to identify patients meeting ‘at risk’ criteria, engage patients in an ongoing pharmacy-based service, conduct the service to meet professional practice standards, negotiate therapeutic goals, and collaboratively devise a management plan with appropriate monitoring parameters. |
