The Primary Health Care and Community Nursing Stream offers effective and specific education to prepare registered nurses for a leadership role in primary health care and community nursing. Key elements of the curriculum include critical thinking, professional accountability, evidenced based practice, leadership, and the development of competency in assessment skills, clinical decision making and clinical judgement at an advanced level.

A graduate of this course can:
1. graduates of the course can demonstrate specialised theoretical knowledge and advanced competencies expected of Primary Health Care and Community nurses; demonstrate knowledge and skills to prepare nurses for professional leadership in Primary Health Care and Community nursing; possess an educational qualification that is endorsed by industry partners, Singapore Health and other key stakeholders as an accepted education course for Primary Health Care and Community nurses
2. graduates can apply discipline knowledge, principles and concepts in accordance with relevant practice codes, standards and legislation; critically appraise research findings and incorporate evidence based knowledge into advanced Primary Health Care and Community nursing practice; participate in research; apply critical thinking and reflective practice to Primary Health Care and Community nursing practice to generate innovative solutions and advance practice
3. access, evaluate and synthesise information relevant to Primary Health Care and Community nursing and the promotion of optimal outcomes
4. communicate effectively and work collaboratively with clients, carers andthe interdisciplinary health care team
5. utilize technologies safely and appropriately for the diagnosis and management of clients and information
6. demonstrate responsibility and initiative in advancing professional development
7. recognise and apply international perspectives to Primary Health Care and Community nursing practice
8. demonstrate cultural competence in the care of all clients, families and communities
9. demonstrate professional, legal and ethical accountability within advanced Primary Health Care and Community nursing scope of practice
