Syllabus: Student nurse's role within the health care team in a community setting. Therapeutic use of self, communication and interventions. Therapeutic nursing interventions within the community. Problem-solving in nursing: Nursing process; assessment, planning, implementation, evaluation (clinical decision-making) discharge planning. Using best practice to plan and implement nursing care within the community. Professional knowledge within a community setting. Australian Nurses and Midwifery Council scope of practice as it applies to the nursing process, care plans/applied nursing care, documentation. Partnerships in care within the community (model) including chronic care model. Collaborating with the interprofessional health care team. Integrated health care including mental health. Use of the ecological model to deal with nursing problems.
