Syllabus: This unit describes the role of the pharmacist within the multidisciplinary health care team in educating patients in self-management of their chronic disease state through development of individual management plans (including drug and non-drug therapiesand monitoring of outcomes). Also includes, review of patient chronic disease progress and modification of management plans where appropriate.
