Syllabus: This unit involves the review and management of drug-related problems in high risk populations including the elderly, those with chronic disease and Indigenous Australians. Includes: commonly encountered drug-related problems and how they contribute to negative patient outcome; strategies to resolve drug-related problems through medication management review for maintenance of health; medication management review programs in Australia and other countries; documentation of the medication management process to assess drug-related problems and to develop actions and recommendations.
