Syllabus:Clinical pharmacy and pharmaceutical care. Infrastructure and personnel requirements. Patient monitoring. Principles of prescription review with respect to monitoring drug therapy. Establishing patient relationships. Effective and comprehensive patient counselling. Patient compliance. Marketing pharmaceutical care services. Evaluating therapeutic outcomes. Documenting pharmaceutical care services. Developing pharmaceutical care plans.
 
Unit Outcomes: On successful completion of this unit students will have - Demonstrated an understanding of the philosophy of pharmaceutical care and the components required for its implementation. Demonstrated an understanding the infrastructure requirements for the provision of pharmaceutical care and the role of pharmacy personnel within the pharmaceutical care model. Established patient-pharmacist relationships essential to the provision of pharmaceutical care. Developed the capacity to obtain and maintain patient medication histories. Demonstrated familiarity with factors that require monitoring in a patient to ensure optimal drug therapy. Develop the skills to counsel patients on relevant aspects of their drug therapy. Developed strategies to market pharmaceutical care services. Demonstrated an understanding of the process of documenting pharmaceutical care, including patient care plans based on desired patient outcomes. Demonstrated an understanding of the barriers to patient compliance and be able to develop strategies to enhance patient compliance. Developed an understanding of the roles and responsibilities of other health care professionals and can effectively communicate with these professionals and participate as part of the health care team.
